PHOTO AND INFORMATION SUBMISSION CONSENT

By checking this box and submitting photos, I, as the parent or legal guardian of the child in the submitted photograph(s), give my consent to StoryBox Entertainment LLC (operating as “Thumb Follow Me”) for the following:

PHOTO AND VIDEO USAGE CONSENT:

I grant permission for StoryBox Entertainment LLC to use, display, publish, edit, alter, copy, exhibit, and distribute photographs and/or video recordings of my child for promotional, educational, and entertainment purposes across all media platforms including:

• Thumb Follow Me website and interactive games
• Social media platforms (Facebook, Instagram, YouTube, TikTok, Twitter, and others)
• Video productions and content
• Marketing materials and advertisements
• Print and digital publications

PERSONAL INFORMATION CONSENT:

I consent to the collection, storage, and use of my child’s first name, age, and state/country of residence in connection with their submitted photograph(s) for identification purposes in videos, social media posts, and website content.

IMPORTANT NOTICE ABOUT SUBMISSIONS:

I understand that submitted photos may or may not be featured in any video, social media post, or other media content. If my child’s photo is selected for use, it may take many months before the submission appears in any published content. Not all submissions will be used, and StoryBox Entertainment LLC has sole discretion in determining which photos to feature.

TERMS OF USE:

• All submitted photographs become the property of StoryBox Entertainment LLC and will not be returned
• No royalty, fee, or other compensation will be paid for the use of my child’s image or information
• This consent remains in effect indefinitely unless I revoke it in writing
• I may revoke this consent at any time by contacting StoryBox Entertainment LLC, though content already published may remain in circulation
• I release StoryBox Entertainment LLC from any claims related to the use of my child’s image or information as described

LEGAL CONFIRMATION:

I confirm that I am the parent or legal guardian of the child in the submitted photograph(s) and have the legal authority to grant these permissions. I have read and understood all terms of this agreement.

This policy complies with applicable privacy laws including COPPA (Children’s Online Privacy Protection Act). StoryBox Entertainment LLC is committed to protecting children’s privacy and will use submitted information only as described above.
